EST Updates: SLP SOAP note guide with lots of examples

Hi Reader,

I took a deep dive into what actually makes a SOAP note clear, defensible, and quick to write. It's been a labor of love (or desperation?), but it's ready!

The free download is long, but it's easy to skim. I included guiding questions, verb banks, sentence starters, and examples.

If you'd like to write your session notes faster or more succinctly, this guide just might help you!

🧠What I'm working on

Almost all of my blog time was dedicated to the SOAP note guide, but I did figure out the print margins on the spiral bound book and made myself a cheat sheet.

It looks like most of my handouts are within the margins, but I'll find out for sure as I update the footers.

Write SLP SOAP notes better and faster (with examples). If you read the post, you'll hear about my documentation woes (some of them!). But, long story short, I wrote this guide to help me write better (and shorter) notes.

The handout (SLP SOAP Note Guide) is long, but it's designed to be easy to scan. Feel free to share it with your colleagues.

Either way, you will find a guiding question, what belongs, helpful verbs, sentence starters, and example statements for each SOAP note section.

In fact, you'll find 5 separate sections for Assessment!

Why would I do something so crazy?

Ever have trouble articulating the skill you provided during a session? (Please tell me I'm not the only one!)

Turns out, there are 4 types of clinical reasoning. (Maybe more?)

Before I wrote this guide, I would not have been able to name them off the top of my head.

So, I wrote a guiding questions, what belongs, helpful verbs, sentence starters, and example statements for each type of clinical reasoning.

The 5th section gives examples of statements that combine types of clinical reasoning, since this is how we typically write our notes.

The draft handout has already been helpful, and this final version is even better. I'm excited to try it out this week!

If you try it out, I'd love to hear what you think! Constructive feedback is always appreciated.

Hi Reader,My company doesn't want us to say "patient tolerated" anymore. They want specificity. So I worked with Chat GPT to come up with a bunch of other ways to report how the patient did. I organized it by what we actually mean by "tolerated" since that term can cover a lot! You can find the link to the bonus handout below. 🧠What I'm working on I'm nearly finished with the SLP SOAP note guide. The draft version has really helped improve clarity in my own documentation, and the final...

Hi Reader,I spent an hour or so coming up with a font-size screener for people with low vision. I'm sharing it with you, in case you find it helpful. You can learn how I'm using it in the "Featured post & handout" section below. 🧠What I'm working on First, I'm working on a comprehensive cheat sheet for writing high-value but succinct SOAP notes. (Brevity is a challenge for me, as anyone who has read my blog posts in the past couple of years may guess!) The cheat sheet has been really helpful...

Hi Reader, My current work-related projected is improving my documentation. Currently, I feel like I include too much information and repeat myself too much. So, I spent several hours last week doing research and making a draft cheat sheet. It's helping! Once I've made adjustments and make it easier to scan, I'll share it with you. 🧠What I'm working on With over 130 free PDF downloads, even I forget what I have! I'm working on publishing a spiral-bound book to sell for a modest profit. Here...
